Telehealth Informed Consent

Last updated 5/23/26

All therapy services provided by Erin Reddinger, LCSW-C through Transformative Therapy are delivered via telehealth — that is, through secure, real-time video technology. Before beginning services, all clients are asked to review and sign a Telehealth Informed Consent form. The information below summarizes what that consent covers.

A complete Telehealth Informed Consent form will be provided to you through the client portal prior to your first appointment.

What Is Telehealth?

Telehealth refers to the delivery of healthcare services — including psychotherapy — via electronic communication technologies such as secure video conferencing. All sessions with Erin Reddinger, LCSW-C are conducted via:

Telehealth Platform:  [FILL IN — e.g. SimplePractice, TherapyNotes]

This platform is HIPAA-compliant and uses end-to-end encryption to protect your privacy during sessions.

Benefits of Telehealth

  • Accessible from any private location with a stable internet connection

  • Eliminates commute time and geographical barriers

  • May be easier to schedule around work, family, or health constraints

  • Research supports the effectiveness of telehealth for many mental health concerns, including anxiety, trauma, depression, and relationship issues

Risks and Limitations

Telehealth services, while effective, carry certain limitations that differ from in-person care. By proceeding with services, you acknowledge awareness of the following:

  • Technology failures (internet outages, software errors, audio/video disruptions) may interrupt sessions

  • Telehealth may not be appropriate for all clinical presentations or levels of acuity

  • Confidentiality may be compromised if sessions are conducted in a non-private space or on a non-secure internet connection

  • In the event of a mental health emergency, Erin Reddinger, LCSW-C may have limited ability to provide direct assistance and will coordinate with local emergency services

Your Responsibilities as a Telehealth Client

  • Conduct sessions from a private, secure location where you will not be overheard

  • Use a stable internet connection and a device with a working camera and microphone

  • Ensure your device and software are up to date

  • Do not conduct sessions while driving or in a public space

  • Have a phone available in case of technical difficulties requiring a switch to audio-only

Emergency Protocol

If you are experiencing a mental health emergency during or between sessions, please do not rely solely on telehealth contact:

  • Call 988 (Suicide & Crisis Lifeline) — available 24/7

  • Call 911 or go to your nearest emergency department

  • Contact a trusted person in your local area who can be with you or assist you

At the start of services, you will be asked to provide a local emergency contact and confirm your physical address so that local emergency services can be contacted on your behalf if necessary.
